By repositioning the sagging deep cheek fat (located in a layer deeper than the SMAS) to the hollowed area beneath the cheekbone (submalar hollowness), both the drooping deep cheek fat and the sunken lateral cheek can be improved.

The buccal fat is dissected along with its capsule, spread toward the lateral cheek, and fixed to the periosteum of the lateral zygomatic arch.
